Cover Page: The cover page should include your organization name, organization address, name of project, point of contact name, email address, and phone number.
Organization Background
Organization Name: Legal name of the applying organization.
Contact Information: Primary contact name, phone number, and email address.
Organization Description: A brief overview of your organization, including mission, goals, and community impact.
Project
Project Proposal: Detailed description of the project for which funding is sought, including objectives, target community, and expected outcomes.
Requested Support: Specify the type of support requested (financial, in-kind donations, volunteer support, etc.).
Budget Overview: An outline of the project budget, highlighting how the requested support will be utilized.
Previous Achievements: Information on past projects and their impact on the community.
